Admission Requirements

Prepare the following documents based on the grade level you are applying for.

Kindergarten Requirements

  • Applicant must be 5 years old by December 31, 2026
  • Latest report card from Nursery (if available)
  • Photocopies of PSA Birth Certificate and Baptismal Certificate
  • Two recent 1″×1″ ID photos
Application Fee: ₱200.00 for Kindergarten applicants.

Grades 1–3 Requirements

  • Latest report card (with a satisfactory general average)
  • Photocopies of PSA Birth Certificate and Baptismal Certificate
  • Two recent 1″×1″ ID photos
Application Fee: ₱800.00 for Grades 1–3 applicants.

Grades 4–11 Requirements

  • Latest report card (with a satisfactory general average)
  • Photocopies of PSA Birth Certificate and Baptismal Certificate
  • Two recent 1″×1″ ID photos
  • Two Recommendation Forms (provided by Lourdes School) filled out by two of the applicant’s present/previous advisers, teachers, principals, or guidance counselors
Application Fee: ₱800.00 for Grades 4–11 applicants.
  • Certificate of Recognition as a Filipino Citizen
  • Photocopy of Philippine Passport or Foreign Passport
  • For students from schools abroad: complete academic records from Kindergarten to the last grade attended

How to Apply

Admissions Process

Follow these steps to complete your child’s application for SY 2026–2027.

Complete the Application Form

Download the official Application Form and fill it out completely. The form is available for download above.

Submit the Required Documents

Digital Submission: Email digital copies to admissions@lsm.edu.ph with the subject: “[Student’s Name] Admission for SY 2026–2027.” For recommendation forms, recommenders may send directly to lsmregistrar@lsm.edu.ph.
In-Person Submission: Bring hard copies to the Registrar’s Office. Recommendation forms may be collected from recommenders in a sealed envelope.

Schedule the Entrance Exam

Entrance exams are conducted on the following days and times. Applicants must arrive at least 15 minutes early.

Interview

Applicants to Grades 4–11 will undergo a personal interview scheduled either before or after the entrance exam.

Pay the Application Fee

Online: Fund transfer or bank deposit to BPI Account No. 4161-0040-87 under “Lourdes School of Mandaluyong.” Send proof of payment to lsmcashier@lsm.edu.ph and lsmregistrar@lsm.edu.ph. Include child’s name, grade level, and “Entrance Exam Fee” as the payment purpose.
In-Person: Payment may be made at the school cashier on the day of the face-to-face exam.

Exam Results Notification

Parents/guardians will receive an email with the exam results within 3–5 working days after the exam.
