ONLINE REGISTRATION

ONLINE APPLICATION
  1. Fill-in the Application. Download file here.
  2. Gather digital copies of the document requirements for the Grade Level; the list at www.lsm.edu.ph/admissions. Hard copies of the documents may be digitalized by scanning them or taking clear, HD photos of them.
  3. Send the Application Form and documents to lsmregistrar@lsm.edu.ph.
  4. The Registrar will send information regarding succeeding procedures.
  5. When the applicant is accepted, the Registrar will send his Student Number. This is needed for Online Enrollment.
PARENTS AND GUARDIANS PORTAL REGISTRATION
  1. Go to bit.ly/LSMPGPortal to access the Parents’ and Guardians’ Portal. To avoid technical issues, please use a desktop or laptop computer.
  2. Click REGISTER HERE and provide the necessary information.
  3. Do not close the window or tab, or surf away from the site, when retrieving your Activation Code from your email.
  4. If the Activation Code is not in your email Inbox within a minute or so, please check your Spam folder.
  5. Once you have access to the main page, you may register your boy(s) to complete the process.

TO REGISTER YOUR BOYS

  1. From the Main page, select Guardian Profile Maintenance on the left panel.
  2. Enter your boy’s six-digit Student Number (no hyphen). Wait for your boy’s name to appear (you can click the moving pen icon on the left of the input field if it does not appear).
  3. Select the name of your boy then click the add button.
  4. The Registrar will confirm your details as soon as possible, usually within 24 hours. This is to protect the integrity of our records and your data privacy. When the Registrar has confirmed your details, your boy’s status will be changed to Activated.
RETURNING STUDENTS AND ACCEPTED APPLICANTS
  1. Log in using your registration details.
  2. From the Main page, select View Profile. It will be directed to the Student’s account.
  3. On the left panel, select Enrollment.
  4. Verify the details of the enrollment such as the Student Number, Name, Grade Level, and School Year.
  5. Select the Learning Type and Payment Mode.
  6. Select your response to the Enrollment Contract (and the Non-Catholic Waiver, if applicable to you).
  7. Select Save and Assess button.
  8. Repeat Steps 5 and 7 to change learning type and payment mode.
